Patients need to meet the following eligibility requirements:
• Must be a Mecklenburg County resident for at least 6 months
• Not currently eligible, pending or receiving Medicaid, Medicare or any other health benefits
• At or below 200% Federal Poverty Guidelines
• Not currently a patient of any sliding scale or subsidized health clinic within the past 24 month
If you meet the eligibility requirements, then every effort will be made to provide you with needed health services requested by your assigned physician for as long as you maintain your eligibility, follow the program guidelines and Physicians Reach Out is able to secure the resources necessary to continue this program.
